Libertatem Healthcare are seeking to recruit *Due to our clients needs* an Experienced Female Brain Injury support worker, to support our client based in Mill Hill. Our male client is in his early 60's, who sustained a brain injury.
*Due to our clients needs we are looking to recruit experienced brain injury support workers, who have experience with brain injuries and complex care needs.*
Hours:
12h day shifts Monday - Sunday
Alternative weekends are required.
Responsibilities
As part of her support team you will be responsible for assisting with:
· Supporting our client to access the community
·Full personal care
· Providing development and social activities
· The delivery of his rehabilitation program
· Providing encouragement and motivation
· Help with emotional management.
·Attend Quarterly team meetings
·Work alongside Care Coordinator and Clinical Lead.
·Provide personalised support in a sensitive and confidential manner, to individuals and families according to their personalised support plan
· Participate in monitoring and review of Service Users’ care plans
· Work in a professional manner with all members of the Service user’s Multidisciplinary team and other health professionals that may be involved.
· Promote self-care and self-management to achieve a maximum level of independence for each individual.
Training & Background Checks
An enhanced DBS check & full training will be provided prior to commencing work with our client and you will also be supported by our clinical lead nurse for the area.
